- CrownsPlacement of dental implants has become the preferred choice for tooth replacement. However, an obstacle for some people is time. Traditionally, a patient might wait 8-12 months from tooth removal to final restoration. At our practice, we offer “immediate implants†when there is an opportunity to do so. This means the placement of implant happens at time of extraction, allowing for shorter treatment time (3-5 months shorter). Many times a fixed provisional (temporary crown) can be placed on top of the immediate implant so you never have to go without a tooth. - BridgesSixty implants were placed in 55 patients at six centers according to a predetermined protocol. All implants were placed in extraction sockets and were subjected to immediate temporization and function. Definitive prostheses (58 single crowns and one two-unit fixed bridge) were placed within the first year. Clinical and radiographic examinations were performed at implant placement and after 3, 6, 12, 24, and 36 months. Measurements of implant stability, papilla index, plaque, peri-implant mucosa, and marginal bone levels were recorded at each visit.
- Root Canal TreatmentBone Grafts When placing dental implants, there must be adequate bone available to support the chewing forces of the teeth. Adequate bone is also necessary to achieve an esthetic result. It is much easier to preserve a patient's existing bone than to reconstruct lost bone. However, patients who have infections resulting from root canal problems, cracked teeth or gum disease can be losing bone in the affected area even if they are not in any discomfort. The longer an asymptomatic infection is allowed to persist, the more bone that is lost. The deficiency of bone in the area of a future dental implant may require the use of a bone graft at the time of implant placement. There are a number of bone graft materials available, and each is selected based on the patient's individual case. The photograph and associated x-ray below illustrate a case when bone grafting would be necessary at the time of extraction and implant placement.

- Bone GraftingBone loss as a result of uncontrollable periodontal disease is illustrated in the above x-rays. Early extraction was indicated due to the aggressive nature of the periodontal disease in this case. Implant placement gets more difficult as more bone is lost. In patients who smoke, aggressive treatment including extraction and implant placement may not be indicated. Bone grafting may be helpful in anchoring the implants, but can result in a compromised esthetic result as shown below
