- Breast ReductionFemale Breast Reduction. There is perhaps no operation that is more poorly treated in the world of health insurance medicine. Over the past two decades I have watched what was a commonly performed and life enhancing operation become more and more difficult to get covered by insurance. Requirements for grams removed has gone up; criteria for selection have become more and more strict. The most frustrating reason for denial is when the insurance companies ask patients to lose weight before breast reduction surgery will be approved. It has been my experience that reduction of weight on the chest wall improves posture, muscle tone, and the ability to diet and exercise. Breast reduction, perhaps more than any other operation that I perform, is often is followed up with the statement, "I wish I had done this years ago".
- Breast LiftSometimes patients are confused about when to consider breast lift. My thinking is this: If you like the way you look in a bra, you like your breast size, and do not want to be larger, but when the bra comes off your breasts go south you are likely a good candidate for a breast lift.

- AbdominoplastyFor the patient who is having abdominoplasty (tummy tuck) at the same time as breast augmentation hollow or trends abdominoplasty breast augmentation can be performed in some cases. This involves the placement of sub muscular breast implants through the abdominal plasty incision. I offer this procedure selectively to appropriate candidates. It can only be performed with saline implants because the approach is more difficult there can be a higher risk of bleeding asymmetry with this procedure.
- Breast SurgeryCongenital inverted nipples are a common problem. They can easily be corrected under local anesthesia and can also be treated in time of breast surgery, reduction augmentation and lift. In order to release the nipples, a small incision is made at the base of the nipple and the breast ducts wish tethered the inverted nipple are released. Because release of the nipple involves release of the underlying ducts breast-feeding can be affected. If an inverted nipple is a new finding it must be evaluated to make sure that there is no underlying breast cancer before surgery.
- Scar RevisionEven in this population scar revision will sometimes improve results because scar revision involves the removal of only a small amount of tissue. In breast lift, reduction, and even sometimes augmentation scars can widen because of tension on the skin. Scar revision is generally performed about one year after surgery when the tissues have relaxed and tension is minimal. In this situation reapproximating the scar can sometimes give a better result. For small scars this can be done on the local anesthesia.

- GynecomastiaMen may also feel afflicted by enlarged breasts caused by a condition known as gynecomastia. Gynecomastia can be an embarrassing condition. Many men with the condition are embarrassed and always wear a shirt. There are differing treatments for male gynecomastia depending on the age of the patient and the degree of skin laxity. Tighter skin with gynecomastia may in some cases be treated with liposuction alone or liposuction with limited incisions to remove denser breast tissue. If skin is loose or droopy reduction may require skin excision. Because our goal in this surgery is always to minimize the need for scars Gynecomastia is best treated in its early stages when the skin is more elastic.
