In some situations, return to pre-motherhood figure occurs over the 3 to 6 months following delivery. However, many women express frustration and dissatisfaction when they’re unable to recover their prepregnancy shape, despite appropriate diet and exercise. - MammographySignificant advances have been made in the management of breast cancer over the last 20 years. Improvements in diagnostic techniques and protocols have resulted in progressively earlier detection of breast cancer, which is the most important factor mitigating towards effective treatment and potential cure. Women who carefully follow recommendations for regular breast self exams, periodic breast exams by their gynecologist or family physician and routine screening mammography, benefit from potential diagnosis of early or ‘minimal’ breast cancer and are then eligible for breast conservation treatment. However, for those less fortunate women whose breast cancer is more advanced at the time of initial diagnosis or for whom breast conservation treatment is not appropriate, a modified radical mastectomy may be the optimal treatment. - ChemotherapyDelayed post-mastectomy breast reconstruction’ ). When performed as an immediate post-mastectomy procedure, patients benefit from the preservation of a greater variety of treatment options and may be eligible for a ‘skin sparing mastectomy’. Following breast mound reconstruction and the completion of any additional necessary breast cancer treatments, such as radiation and/or chemotherapy, patients complete their breast reconstruction in several additional stages including a possible ‘balancing’ procedure on the opposite breast, nipple reconstruction and nipple/areolar tattooing. Due to the many factors affecting the choices in breast reconstruction as well as the variety of reconstructive techniques available, it is not possible to appropriately outline them all in this forum.
- General SurgeryI recently had the privilege to participate in a two-week surgical charitable mission trip to Liberia, West Africa in March 2019 with an organization called HEARTT (Health, Education, Aid and Research through Training). During this trip, I was accompanied by a nurse and team of other surgeons, one of whom was my son, Dr. Robert M. Kimmel, Jr., a senior General Surgery resident at Waterbury Hospital in Connecticut. During this mission trip, we were able to provide necessary surgeries to impoverished citizens of Liberia, who lack the resources to otherwise afford access to surgical care, as well as provide teaching and interaction opportunities between HEARTT members and the Liberian medical professionals. Additionally, having the opportunity to work side-by-side and share this experience with my son was extremely rewarding. The lack of resources and meager conditions forced our team to be both flexible and resourceful and definitely made me appreciate the standard of medical care we enjoy in the US.
- Wound CareDermabrasion is a procedure employed to enhance the skin surface by improving or eliminating contour irregularities. Performed on as outpatient basis, using a local anesthetic in combination with intravenous sedation,State Licensed, Nationally Accredited Ambulatory Surgical Facility, (see ‘about Keystone Surgery Center’ ) Dermabrasion involves the use of a power skin dermabrader or “sanding wheel” device to gently strip away excess unwanted skin cells, thereby improving deep or concentrated wrinkles, chronic acne scarring or skin surface irregularities resulting from prior injury. Dermabrasion is particularly useful for correcting contour irregularities in the area of the cheeks and around the mouth. Following this procedure, a “brush-burn” type of wound results, requiring topical wound care for about 5-7 days until surface healing occurs. What emerges following this healing process is an enhanced, smoother skin contour and more radiant complexion. This improvement in facial skin contour can, in turn, minimize the need for the use of camouflage cosmetics in women or the embarrassment resulting from chronic acne scarring, thereby enhancing confidence and self esteem.
- Ear SurgeryOtoplasty or ear surgery involves “pinning back” or reshaping protruding ears so they are in the proper proportion and position with respect to the size and shape of the face and head, without altering hearing ability. This surgery can be performed at any age as long as the ears have reached their full size, usually beginning about 5 to 6 years of age. Having this procedure performed at a young age is advantageous in several respects: the cartilage is extremely pliable, thereby permitting greater ease of shaping; secondly, the child will experience psychological benefits from the cosmetic improvement. Additionally, when performed in childhood, otoplasty may represent a ‘covered benefit’ in some health insurance plans. Through an incision concealed behind the ear which heals imperceptibly, otoplasty is most commonly performed on an outpatient basis requiring only brief general or intravenous sedation anesthesia, State Licensed, Nationally Accredited Ambulatory Surgical Facility. - Hand SurgeryAs a plastic surgeon with a broad scope of practice including cosmetic, reconstructive and hand surgery, the demands on my time and energy can be significant. But I never find this burdensome because of the deep satisfaction that comes from surgically intervening in the lives of others to eliminate suffering, restore function, and even enhance appearance. In order to sustain the focus and concentration necessary in my field, however, I feel it is extremely important to allocate time for regular exercise and recreation. So when I am not working, I typically enjoy tennis, hiking, biking, swimming, golfing, skiing and exercise classes as my primary forms of aerobic conditioning. I also enjoy music, particularly if performed live, and movie viewing. I am an avid Philadelphia sports fan following the Phillies and Flyers, but I am particularly rabid when it comes to the Philadelphia Eagles. - Face LiftA thread lift is a simple procedure where Dr. Robert Kimmel uses temporary sutures to produce a subtle, but visible lifting of the facial skin. Unlike a surgical facelift, where excess sagging skin is trimmed and underlying support tissues brought back to a higher position on the face, a thread lift uses sutures to simply suspend the skin by stitching up portions of it. This has the effect of pulling the skin back slightly and lifting and tightening the face.
- Neck LiftDepending on this assessment, the optimal recommended treatment may be neck lift, liposuction, neck lipectomy, platysmal tightening or a combination of these techniques. Specifically, if skin and muscle tone are adequate, liposuction alone may suffice to remove excess fatty tissue. When a potential patient comes in who has not lost all of her baby weight yet, we advise her to come back when she has lost all of the weight.
