      Based in Port Jefferson Station, N.Y., Woodhaven Nursing Home is a privately owned and licensed by New York state and is a certified Medicaid and Medicare facility. It has been providing health care services to its residents for more than 25 years. It offers a full continuum of care with a variety of programs that include a skilled nursing facility providing long-term care, subacute care and short-term rehabilitation, an assisted living program, adult home and a licensed home health care program. In addition, Woodhaven Nursing Home provides various therapeutic recreational activities for all residents that include team and individual games, arts and crafts, movement therapy, discussion groups, reminiscence sessions, creative writing, short story and poetry writing, pet therapies, musical sessions, movies, entertainments, lectures, community outings, community involvement groups and religious services.
