      Since being founded in 2004, Evolution Home Physical Therapy, has built a team of physical and occupational therapists conveniently throughout Long Island and serves most parts of Nassau, Suffolk and Eastern Queens. Our goal is to provide the best care possible for each patient seeking relief from injury or chronic illness. We carry out detailed consultations with each client to determine their health history and assess their goals for the future. Our team works together to share knowledge and experience in order to support each other to better care for their patients. We offer a wide variety of treatment options including home care physical and occupational therapy services for those not able to leave their homes or those with extremely limited mobility. We have years of experience helping patients recover from falls, injury, hospitalizations and many other disabling conditions.
