I WAS IN YOUR DENTAL OFFICE TODAY, 4/2/2015 at 10:40AM-11:00 FILLING UP TONS OF PAPER WORK for temporary DENTURE due to an emergency that the jacket you installed 2010 fell off. Unfortunately, I went WITH A HUMANA FITNESS CARD, ALL DETAILS WERE THERE NEVERTHELESS, I WAS ASK TO GET MY HUMANA ID CARD AT home. When I got home called HUMANA cause the clinic does not know HUMANA phone number. Therefore, I request a new card today because I lost it.



THIS IS TO DOCUMENT THAT I WAS NOT TREATED AND TERRY THE SECRETARY DOES NOT KNOW THE HUMANA PHONE NUMBER in spite that the card I have for fitness has a HUMANA PHONE NUMBER. With 1-800 NUMBERS FOR VERIFICATION AND CHECK, OF INSURANCE COVERAGE,



I WENT HOME AND CALLED MY INSURANCE, AND THE HUMANA REPRESENTATIVE SAID THE CLINIC KNOWS THE NUMBER AND I SHOULD NOT LEFT the clinic.



I BELIEVE MY INSURANCE BUT CALLED the clinic BACK AND WAS TOLD THE APPOINTMENT IS CANCELLED, E-MAIL WILL BE SENT TO MY ACCOUNT AT [email protected]



HOPE THE CLINIC WILL NOT CHARGE ME AND MY INSURANCE, HUMANA FOR THE DAY.



FOR DOCUMENTATION AND PROTECTION OF THE PATIENT FOR ANY UNTOWARDS INCEDENT AND TO PREVENT ANY POSSIBLE ISSUE THAT MAY ARISE OUT OF THIS CIRCUMSTANCES.

Doctor Olinits is AWESOME!! My family has been going there for many years. The staff is welcoming, friendly and compassionate. Dr. Olinits listens to his clients concerns and addresses them accordingly. I am always pleased with the service, professionalism and courtesy from everyone, whether it is a cleaning, root canal or a more extensive procedure. My 5 year old son just had a filling on his tooth and was fine that evening! I would recommend Dr. olinits and staff to anyone looking for a dentist.
