Personalized care in the home is made possible through Phoebe Home Care. Services are provided on an individual basis by health care professionals in the comfort of your own home. Guidelines to Receive Home Health Care In order to receive home health services, the patient must be homebound. A homebound patient is defined as one who requires considerable effort and assistance to leave home. Medicare guidelines require that the care given must be medically reasonable, necessary and ordered by a physician. The patient must require skilled care that can be administered intermittently. What must I do if I have a need for home health care? Contact your physician or social services department of your local hospital.
