      home:: about MHG NOTIFICATION Effective May 1, 2009: MHG will require a $20.00 payment for the completion of all Disability and FMLA forms. Payment will be required at the time the forms are received. About Us Michigan Heart Group was established in 1991 by David R. Cragg, M.D., F.A.C.C. We have spent the last several years creating a unique health care experience for our patients. Michigan Heart Group encompasses every aspect of cardiovascular health care, this side of surgery. We specialize in clinical, diagnostic, interventional cardiology, vascular medicine and electrophysiology. Our goal is to provide you with the most personal care, up-to-date assessments and treatments to keep you young at heart. Effective April 2003 the federal law will require Michigan Heart group to adhere to the Health Insurance Portability and Accountability Act of 1996 (HIPAA). This Act is about standards for automating the business side of health care, electronic transmission and processing of claims, payments, enrollment, and eligibility. The Notice must explain what protected health information is (PHI). It also must include examples of how MHG might use and disclose PHI. The Notice will inform patients of their rights regarding medical information. Within the Notice the Compliance Officer will be identified. The Notice will be posted on the Web Site and at the check in and check out terminals of the Patient Services area. Click here to access the notice.
