Welcome to Stephen A. Cook, DDS, MSD! We are a locally owned and operated dental clinic specializing in periodontics and dental implants. We offer services that include dental implants, dental hygiene, dentures, crown lengthening, bone grafting, tooth extraction, and much more. We work together with you and your family dentist to help you establish and maintain lifetime dental health. Call us today. About the Doctor: Dr. Cook attended Purdue University for undergrad study in physics and pharmacy. He received an early admission to Indiana University School of Dentistry at the Med Center in Indianapolis and graduated with honors in 1978. He subsequently spent four years in the United States Air Force as a dental officer. After completing his service in 1982, he began a two-year post-graduate residency program, earning a Certificate of Specialization in Periodontics, as well as a Master of Science in Dentistry degree in June, 1984. He opened his periodontal and implant surgery practice in Lafayette in September, 1984 at its current location and has been pleased to serve the periodontal and implant-related needs of patients in the Greater Lafayette area for over thirty years.
