Signature Health Care of Jacksonville

I was a patient for almost 4 moths in this facility and for the most part it was a nice experience. Most of the staff where very nice and helpful addressing the needs for my recovery and the return back to a normal life at home with my family. The physical therapists were great especially Matt for pushing me when I really wanted to give up. The few thing's that made me rate at 3 stars was they had constant problems providing fresh bed linens and switching staff around too often. When it was coming time for my discharge I expressed my concerns of not having adequate medication to be able to setup for primary care physician and I was only given 4 days supply on some of my meds when discharged. Most doctors offices now days takes 4 days just to talk to someone to make an appointment let alone get there and be assessed. I was also supposed to get some home equipment (wheelchair,hospital bed) but they failed to arrange these thing's.The case worker was the main reason for these thing's.
