A hospitalist is a physician who specializes in the practice of hospital medicine. Once you are discharged by your Monarch Hospitalist, you will need to contact your primary care physician for further treatment. Hospitalists and SNFists care for patients when they are in facilities. They do treat in a clinic or practice like primary care physicians.
