Chatham Oaks is a private, nonprofit agency that serves people 16 years and older with mental and physical disabilities. Its nursing staff implements individualized health care plans for each resident that includes medication management, physical and mental health assessment, physical and speech therapies, and individual and family counseling. It offers support and guidance to residents who are involved in work programs, as well as to those who are in transition toward independent living. The agency s respite care program offers short-term relief to families with mental disabilities, developmentally disabled and brain injured members. It also provides opportunities for its residents to participate in a variety of recreational programs, such as arts and crafts, needlework, weaving, woodworking, and volleyball and basketball games.
