Community Health Pavilion East is a health pavilion on the eastside of Indianapolis, IN providing patient care and medical services. Services offered at Community Health Pavilion East include Community Physician Network primary care physicians and a patient-centered medical home, diagnostic testing (lab and x-ray), osteoporosis testing, breast care services (mammography), and physical therapy and rehab. The pavilion is also home to the Community Hospital East Family Medicine Residency Program.
