- Primary CareA. We will need a Medical Records Release Form completed and either faxed, mailed or brought to the office. By law, we are required to provide your records within 30 days if they are stored onsite or within 60 days if they are stored offsite, however, typically we complete your requests sooner. If you are requesting your records be sent directly to another provider for collaborative care, there is no charge. If you are requesting a copy for any other purpose, there will be a charge. A Medical Records Release Form is not required for us to send your records to your primary care provider or a provider to whom we have referred you. If you have any questions, please contact our Medical Records Department.
- Family PracticeStephanie has been a Nurse Practitioner since 1996. She is Board Certified in both Family Practice and Women’s Health. Stephanie began her formal nursing education at Xavier University for undergraduate studies and became a nurse in 1992. She continued her education to obtain a Master of Science in Nursing as a Family Nurse Practitioner from the University of Cincinnati and a Post Master’s Certificate in the Women’s Health Nurse Practitioner Program from The University of Michigan. In 2016, Stephanie completed her formal education with a terminal degree by obtaining a Doctorate of Nursing Practice Degree from Walsh University. She is also a certified Menopause Practitioner from The North American Menopause Society and a CPR and First Aid Instructor from The American Safety and Health Institute. She holds membership in multiple associations including the Association for Advanced Practice Nurses and the International Association of Nurses. Stephanie enjoys mentoring Nurse Practitioner students and has served as faculty for two local Universities. She is a native of Canton where she resides with her husband, Mike. In her spare time she enjoys spending time with her family, traveling, golfing, skiing, running, yoga, kayaking, camping, and just about anything outdoors!

In 1990 she earned a Master of Science in Nursing with a Certificate in Midwifery from Case Western Reserve University. She is a board certified Nurse Midwife with 24 years of experience in Nurse Midwifery practices in North and South Carolina. She is a member of the American College of Nurse Midwives, and the Northeast Ohio ACNM affiliate. While practicing as a CNM in North Carolina she served as North Carolina Chapter Chair, Legislative Liaison and Member of a number of North Carolina State task forces on maternal and fetal well-being. Louann served as a member of the Advance Practice Nursing Coalition in the State of North Carolina for two years. In 1993 she was one of the founding mothers of the midwifery service at Catawba Valley Medical Center in Hickory, NC where she attended the births of thousands of babies and taught hundreds of students in the last 20 years. She is proud of the fact that the midwifery practice continues today helping to provide the hospital with a Five Star rating in Obstetrics, Magnet Hospital designation for the last 9 years and a community reputation for excellent care. Louann is married to her husband Tim and enjoys spending time camping and enjoying family activities with her eight children and two dogs.
