General Info

Hello, I'm Amy Poliak, a pediatric nurse practitioner for Affinity Medical Group. While treating pediatric patients, my goal is to build relationships with families and help my patients develop a healthy lifestyle early on. I have a special interest in sports medicine/concussion care;and adolescent health, and look forward to working with new parents and patients. Outside of the office I enjoy running, skiing and camping.
