      Country Estates of Agawam provides sub-acute medical and short-term rehabilitation care. The facility is not able to handle the intensive services of an acute care hospital. The facility maintains the quality and standards of a care facility. Country Estates offers a wide range of skilled nursing and rehabilitation services, including specialized programs, orthopedic rehabilitation, pulmonary, stroke and neurological rehabilitation. The sub-acute programs include wound care, oncology and hospice care, cardiac recovery and nutritional management. Other programs include respite care, counseling and support groups, family education and 24-hour skilled nursing care. Country Estates is Medicare/Medicaid certified, JCAHO accredited and accepts most insurance and managed care plans.
