      Founded in 1965, Pavilion is a long-term care, skilled nursing facility and is located in Sidney, Ohio. It provides 24-hour assistance with activities of daily living, such as bathing, dressing, eating, mobility, hygiene, grooming and toileting. It offers physical, occupational and speech therapies combined with nursing, social services and medical direction in rehabilitation program. Its intermediate and long-term care program assists residents in leading personally fulfilling lives. It offers short-term stays with daily care in a home-like setting and provides food, transportation, housekeeping and laundry services. Its hospice care team members assist with funeral arrangements and support, as well as counsel the family during the bereavement period. Its other facilities include therapeutic diets, bed and bath linens, chaplain, pharmacy, beauty salon and barbershop.
