      The Northwest Continuum Care Center is a nursing center that provides a range of medical services, including short-term rehabilitation and long-term skilled and episodic medical care to meet the individual needs of its residents. It offers daily nursing and pharmacy care, dietary consultation and social services. The center can accommodate up to 70 residents and offers semiprivate rooms. It provides physical, occupational, speech and language therapies. The Northwest Continuum Care Center has a rehabilitation team that creates individualized treatment programs for residents in consultation with their physicians. It is a part of Kindred Healthcare, which operates several long-term acute care hospitals, skilled nursing centers and a contract rehabilitation services business through its subsidiaries across the United States. The Northwest Continuum Care Center is located in Longview, Wash.
