      Park Avenue Extended Care Center provides rehabilitation programs and assisted living services to senior citizens and patients suffering from cognitive impairments, and Alzheimer s and dementia disease. The health care services offered by the center include pain and wound management, respiratory and respite care, skilled nursing, tracheotomy dialysis, parental feeding and long-term care. The amenities offered by the center include gourmet dinner facilities, beauty parlor, barbershop, garages, and one-and two-bedroom apartments with cable television and fully equipped kitchens. It also provides personal care services, such as housekeeping, maintenance, medication management, laundry and transportation. Park Avenue Extended Care Center also provides physical, occupational and speech therapies, and is located in Long Beach, N.Y.
