Been to the E.R. or in the hospital due to problems with medications? If you do...you are not alone. It is estimated that 50% of the population on medication does not take it properly. It costs the average American approximately $2000 per year in doctor visits due to trouble taking medications. Non-adherence to medications causes 30-50% of treatment failures and an estimated 125,000 deaths per year. An estimated 3 million older adults are admitted to nursing home due to medication related problems. Our goal at Medication Management Services is to help clients obtain optimal health while remaining in their home by assisting them the with proper management of their medication, physical assessments, and education. We do this by providing in-home services by an experience registered nurse. You will have scheduled visits where you will have a physical assessment, receive education about your medications, your ailments, and general information on how to manage your diseases. Your nurse will work closely with your doctors and pharmacists to ensure that your medications are appropriate for you and that you do not have any potential interactions. A medication delivery system will be put into place that is tailored to your needs that will make taking your medications easier for you.
