      Established in 1916, Community Nurse & Hospice Care is located in Fairhaven, Mass. It is a private nonprofit visiting nurse association offering a wide variety of home health care and hospice services. The association offers various hospice care services, including medical consultation, skilled nursing, medical social work, pastoral care counseling and bereavement support. The center offers physical, occupational and speech therapy to it patients. Community Nurse & Hospice Care s home health services includes medical social, nutrition and geriatric health counseling; psychiatric nursing; home health aids; and communicable disease follow-up. The center has an immunization, blood pressure control and well-baby clinics. Additionally, it has a wellness center offering yoga, dance, cooking-for-one classes and fitness programs.
