Central New Hampshire VNA & Hospice provides a comprehensive range of services that enable people to receive health care, health promotion and disease prevention, hospice care, clinical services and other support services in the comfort of their home or other community settings. This is accomplished by an interdisciplinary team approach to comprehensive care that includes nursing, therapeutic, medical, psychological, social, spiritual, health education, disease prevention, and bereavement services. Formed through the merger of Community Health & Hospice in Laconia and the VNA & Hospice of Southern Carroll County in Wolfeboro, the Mission of Central New Hampshire VNA & Hospice is ?Promoting dignity, independence, and well-being through the delivery of quality home health, hospice and community-based care services.? Central New Hampshire VNA & Hospice offers in home healthcare services to Lakes Region communities by providing Home Care (nursing services in the home), Rehabilitation Services, Pediatric Care (direct health care, education and support services for children and families), and a comprehensive, team-based Hospice program. Central New Hampshire VNA & Hospice is a not-for-profit, Medicare-certified provider of home care and hospice services, licensed by the State of New Hampshire. The agency is governed by a volunteer Board of Trustees and supported by private and corporate donations. Registered Nurses provide support, care, and education to members of our community who need disease or medication management, wound care, and IV therapy. Licensed Nursing Assistants provide personal care for our clients who need assistance with bathing and dressing. Telehealth remotely monitors a client?s health and helps patients gain greater control of their illness. I.V. Therapy allows the delivery of medicine, such as antibiotics, in the relative comfort of their own home. Physical Therapy provides services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients with injuries or disease. Treatment often includes exercise for patients who lack flexibility, strength or endurance. Prehab An evaluation in your home by a physical therapist prior to your surgery to maximize knowledge of exercise, equipment needs, safety and plans for recovery. Home Safety Evaluations for clients of Physical Therapy, a home safety evaluation assesses the client?s home, making suggestions on how changes can be made to reduce the risk of falls. Occupational Therapy is skilled treatment that helps individuals achieve independence in all facets of their lives. Medical Social Services assist both Home Care and Hospice clients with long term care issues, advance directives, caregiver stress and financial concerns related to illness. Hospice is a special way of caring for people and their family at the end of life, focusing on the patient?s needs and wishes-and on the needs of the grieving family. Bereavement Services support families as they cope with their loss and adjust to life without the physical presence of their loved one. Spiritual Care helps support the patient and family?s search for hope, meaning and reconciliation. Community Clinics include Blood Pressure clinics, Foot Care Clinics and Flu Immunizations. Pediatric Home Visits by Pediatric Registered Nurses for children and adolescents with acute or chronic illness or injuries
