Slepian - Attorney at Law Eric Slepian received his bachelors degree in Business Administration from Pace University, Pleasantville, New York in May, 1984 and graduated Albany Law School of Union University, Albany, New York, in May, 1987. Eric Slepian is a member in good standing of the Arizona State Bar, New York State Bar, and Connecticut State Bar. Eric Slepian is admitted to practice before the United States District Court for Arizona and the Eastern and Southern District Courts for the State of New York. You need to contact your treating physician and have them send you a medical statement explaining why you can't work. Make a list of all of the physicians that have treated you since you became disabled. You need their names, addresses and telephone numbers. Dates of treatment are helpful if you have them, or the date that you began treatment with each physician. Include any hospitals that you have been in or any outpatient therapy that you have received. Make a list of all of the types of work that you have done in the past 15 years. Please provide a job description, not a list of your employers. If you have worked as a plumber for 15 years but for 3 different companies, you would only list plumber, not the individual companies. When you have obtained the application from Social Security or a denial letter, call us toll free for an appointment. We will sit down with you and evaluate the case.
