Tests ordered by physician on numerous occasions not received until family continuously asked. Let in 45* heat during power outage for 3 days, then got pneumonia that was let go for 10 days. Patient left in urinated clothing for hours. Bruise marks on face, neck and ears in long lines as if from fingers; staff blamed on medication side effects. Other patients medications left in room for days. Feces finger prints on medication tubes left on tray where food and beverage is held in room. Patients medication left unattended on nurses station counter. Unexplained broken eye glasses that were then conveniently lost. Patient very depressed and withdrawn. Different stories from staff on medications, orders, amount of food eaten, change in behavior,etc. Clothes worne for 5+ days. No shower for more than 2 1/2 weeks with extreme scaling on skin. Not shaved unless family shaved. Missing articles of clothing. Entire complex smell very badly of urine and  (Hide)
