The Care Consultant is a nurse who serves as your personal medical advocate and liaison to you or your loved one while in a medical facility.Although medical facilities supply nurses and other healthcare workers to care for you or your loved one it is simply not enough. This has been proven by an increase in medication errors , deadly falls, slow recovery rates , misdiagnosis, neglect and even patient deaths. Personnel shortages, the increased use of medical staffing companies to supply workers who are not familiar with patients, high patient to nurse and aide ratios and burnout are many times the cause of poor care and fatalities. Goal: " style="color:#2a2a2a;font-size:large">1.To make sure you are aware of what is going on: that you know your condition, are in agreement with the treatment plan and that everything possible is done to produce the desired outcome. 2. To explain things in terms that you can understand. 3. To review your medications : To make sure each medication is best for your condition, review side effects, drug interactions and warnings specific to your medical condition, to offer alternatives when appropriate and approved by you to the rest of your healthcare team. 4. To be involved in every aspect of your care : diet, intake and output, therapies and restorative care, pressure sore prevention, activities of daily living, medications, activities, psychological care, lab work and testing, etc. 5. To work with the facility healthcare team; doctors, nurses, aides, therapists, dietician and supervisors. 6. To promote health and to make certain that you or your loved one are safe, happy and receiving optimal and Excellent Care.
